Appointment requests

When you submit a request, we collect your name, email, phone number, home visit address, your dog’s details, and any notes you choose to provide. We use this information to respond to you and coordinate care. Please do not include payment details or other sensitive information in the notes.

Requests are stored in our Cloudflare-hosted database and sent to Dr. Sam through Resend, our email delivery provider. The database also records when you submitted the request and its email delivery status. If email delivery fails, the saved request helps us investigate the problem.

Website request records are automatically removed after 90 days. Copies held in the practice’s email system or clinical records are managed separately. To ask about your information or request its removal, email drsam@hearthsideanimalhospital.com.

Website services

Cloudflare hosts this website and processes connection information to serve pages and limit abusive requests. We do not store your IP address in the appointment database. Google Fonts receives browser requests to provide the website’s typography. We use Google Analytics to understand how visitors find and use this website. Google Analytics uses cookies and collects information about website activity, browsers, and devices. We do not intentionally send appointment form contents to Google Analytics. Learn more about how Google uses information from sites that use its services. This website does not provide patient accounts.

Contacting us directly

Email links open your email application. Messages are processed by your provider and the practice’s email service. An appointment is confirmed only after the practice contacts you.
